Internal hemorrhoids begin above the internal opening of the anus. If they become large enough to protrude from the anus, they become squeezed and painful. Small internal hemorrhoids may bleed with bowel movements. External hemorrhoids appear outside the anal opening. They are usually not painful, and bleeding does not occur unless a hemorrhoid vein breaks or becomes blocked. One must consult surgeon if he has got bleeding or pain during defecation. HERE WE ARE DOING CRYO SURGERY FOR PILES AS WELL AS FISSURE IN ANO C R Y O - S U R G E R Y The utility of cooling effect in alleviating certain Human Physical Distresses is not new in the Medical Science as it has been in use since 5 Th Century BC when the famous Greek Physician Hypocrites used it extensively for relieving body aches and checking Haemorrhage. Even to this day , cold application is considered the primary measure to alleviate Hyperpyrexia . This process of cold application has been elevated from relieving stage to curative stage and Cryo-Surgery is preferred in curing certain morbidities. Inhibition of Neurological Function occurs at - Minus 4-Degree Centigrade but this inhibition is reversible & normal functioning is restored at requisite level of temperature. However, below Minus20 Degree Centigrade, permanent functional inhibition or in other words, Cell Death, takes place. But if the temperature is brought down at the rate less than 1-Degree Centigrade/min., the tissue or Cell can remain functional or alive and can thus be preserved for longer duration. As an example, we take the case of bull Spermatozoa which when frozen at a speed slower than 1Degree Centigrade / min. in Liquid Nitrogen, survive at Minus 196Degree Centigrade for as long as up to Five years. Thus, the rate or speed of cooling is most important and has to be regulated to a required level. Cryo Surgery is nothing but killing of diseased tissues by freezing them at a regulated rate of freezing. Thus through Cryo Process, the Diseased tissue gets frozen till death for subsequent spontaneous removal and simultaneous healing. CAUSES OF CELL DEATH IN CRYO SURGERY With the application of cooling process, ice begins to form in both intra as well as extra cellular compartments. Slow cooling 1 degree centigrade per minute produces large ice crystals in the extra cellular fluid. These crystals grow by drawing free water out of the cells & this increases the salt concentration in the cell that ultimately helps in the complete freezing of the cell as a whole. As stated earlier, the speed of cooling is vital. Faster cooling produces very small ice crystals rapidly both with in the cells, & between the cells, often puncturing the cell membrane & thus causing mechanical destruction of the cell membrane as well as hyper osmosis leading to poisoning & ultimate destruction of the affected cell. Another factor acting in the process of Cryo is the thrombosis of the microcirculation leading to cellular anoxia and ultimate Necrosis of the tissue. HISTOLOGICAL CHANGES AFTER CRYO SURGERY The Cryo process leads to destruction of reticular network. Half an hour after Cryo application the frozen area shows piknotic nuclei & coarsely granular not invariably vacuolated and oedematous cytoplasm with destruction of reticular network. This follows marked increase in the extra cellular spaces which are mostly filled with Red Blood Cells as a result of vascular stasis and damage to small blood vessels. A picture of Ischemic Infarct follows it. After about 6 hours, polymorphs, plasma cells & Lymphocytes infiltrate marginal zone. In the frozen area, there is progressive piknosis, thrombus formation in the vessels, disintegration of the vessel walls with eosinophilic Necrosis. By the 4th & 5th day after Cryo application, a fibrous capsule formed by fibroblastic activity in the marginal zone with fine capillary vessels is formed surrounding the Cryo necrotic tissue. This tissue is removed in two ways by absorption & Liquefaction. Finally small residual scar formation heals it, which is almost invisible. DR. O.P.
